Section B Insurer – The insurance company that covers the vehicle you were either the driver of or a passenger in when the accident occurred.
Notice of Claim & Proof of Loss Form – also known as “AB-1 Form”
Primary Benefits – Any coverage for benefits that you and/or your spouse, or your parent may have through work, school, teams, etc.
If you’ve been injured in a motor vehicle accident and require treatment, you should consult your Family Physician or Physical Therapist within 10 days of the accident to qualify for the Diagnostic and Treatment Protocol Regulation (DTPR).
You are required to have your physician or therapist fill out a Notice of Claim & Proof of Loss Form (AB-1) Form.
By completing the AB-1 form within 10 days you may qualify for physiotherapy and massage therapy treatment at no direct cost to you. Upon discussion with your insurance company you may qualify for Section B benefits up to $50,000.
Conditions covered under the DTPR include:
- Ligamentous Sprains (I, II, III)
- Muscular Strains (I, II, III)
- Whiplash Associated Disorder (WAD)
Injuries classified outside of the above will require you to access your Primary Health Benefits (work, school, etc.) prior to the Section B coverage maximum of $50,000.
Ligamentous Sprains (I, II, III)
Injury to the ligaments that attach one bone to another are defined as sprains.
Symptoms include:
- Pain
- Swelling
- Difficulty with movements
Muscular Strains (I, II, III)
Injury to muscles or their attachments to a bone are defined as strains.
Symptoms include:
- Pain
- Swelling
- Difficulty with movements
- Muscle spasm
Whiplash Associated Disorder (WAD)
Whiplash, also known cervical acceleration-deceleration injury is characterized by a collection of symptoms that occur following damage to the neck, usually because of a sudden and unguarded movement of the head forward and backwards or side to side. Injury to intervertebral joints, discs, and ligaments, muscles, and nerve roots contribute to the symptom picture.
Symptoms include:
- Neck pain and/or stiffness
- Blurred vision
- Fatigue
- Dizziness
- Shoulder pain or pain between the shoulder blades
- Pain in the arms or legs, feet and hands
- Headache
- Nausea
- Ringing in the ears
- Vertigo
- Numbness and tingling
Please bring all your Section B Insurer’s information (including your claim adjuster’s name, phone and fax numbers and your claim number) to your Physician or Physical Therapist’s clinic on your first visit.
